Usually 5% of the total cost.  In some cases in high risk areas 100% may be available

Yes. there are certain cost the applicant must cover.  Such cost include background checks, verification of business entity, credential verification of your chosen staff.  These are incidentals and vary depending on the type of licensing, certification and jurisidction.  These cost are typically ranging from $199 - $299.
